Those identified as having elevated risk through the screening tool and the triage process require the next step — an in-depth assessment of their psychosocial barriers.
Each organisation needs an early psychosocial referral pathway that ensures that those identified as having elevated risk in the triage process progress rapidly to a comprehensive assessment.
How this occurs will depend on the organisation’s approach to triage. Ideally, the referral process should be as streamlined as possible. The triage process may automatically trigger a referral for a comprehensive assessment. In more manual setups, the role of the case manager is pivotal in navigating the referral process.
