By creating a therapeutic "pain story," proponents of cognitive functional therapy claim the approach will break the cycle of pain-related distress and disability for individuals with persistent, non-specific musculoskeletal pain.

Non-specific back pain has no clear, single physical or neurological cause that can be diagnosed and treated. Instead, an idiosyncratic tangle of factors including genetics, physical activities, anatomical characteristics, movement styles, thoughts, habits, emotional responses, social circumstances and unhelpful neurobiological patterns combine to create pain.
